What is Conference On The Move?
Participants are taken into communities, educational environments, or health projects in the community. During a half day delegates will explore these site visits. This is only available for in-person attendees.
Start Time: 12:00 PM
Conference on the Move will be in the afternoon of the first day of the conference, following the keynote session.
Registration
You will have the opportunity to sign up for your preferred conference on the move tour in August 2025. Each community site visit is limited to availability, first person to sign up through the survey, can secure their first choice. Be on the lookout for an e-mail in Early August 2025.

Center for Persons with Disabilities, Penn Dental Medicine

Oral health is a fundamental part of overall well-being, yet many individuals face significant challenges in accessing quality dental care. At Penn Dental Medicine’s Care Center for Persons with Disabilities, opened in 2021, we are dedicated to breaking down these barriers by fostering a more inclusive approach to dentistry—one that ensures all patients receive the care they need, regardless of physical, cognitive, or medical challenges. Our mission is to ensure access to care for thousands of adults with disabilities who urgently require oral health care. The Center ensures that every Penn dental student graduates with an understanding of how to competently and compassionately treat patients with all types of disabilities – cognitive, developmental, disease or injury related.
We invite you to learn how oral health for persons with disabilities can be enhanced through community partnerships, collaboration with teachers, health providers outside of dentistry, colleagues in social services, and even family members. Delegates will learn more about the barriers to good oral health and the consequences of neglect of this important dimension and could be of particular interest to people who work with low resourced communities that have a scarcity of dentists and/or dental care. The role that teledentistry can play in the development of personalized care delivery will be presented. Participants will have opportunity to learn about the delivery of individualized oral hygiene care and gain hands-on practice in the application of fluoride varnish and silver diamine fluoride and their role in disease prevention.

Bucks County Association for the Blind and Visually Impaired
This session is designed for professionals who are interested in expanding their knowledge of the field of Low Vision Rehabilitation. This educational presentation will provide a comprehensive overview of the challenges and needs of low vision patients, the service delivery process, as well as effective strategies for evaluation and rehabilitation.

- Understanding Low Vision – definition, causes and impact on patients’ daily lives
- Service Delivery Process – patient-centered care approaches, including engaging patients and involving family and community support
- Components of Low Vision Evaluation – medical history, understanding visual acuity, contrast sensitivity, visual field testing, functional vision assessment and medical decisions for the best care of the patient
- Low Vision Rehabilitation – use of assistive devices, adaptive techniques and strategies to enhance visual function and independence
In addition to the educational presentations, the session will offer hands-on experience to allow participants opportunities to use various low vision aids and gain first hand experience with the latest tools and technologies available.

Visit one of two LGBTQ+ youth centers that are part of Planned Parenthood’s Rainbow Room. Meet professional sexuality educators who staff the program, for a tour, an engaging overview of services and key lessons in serving this beautiful, inspiring and also targeted community. A Woman’s Place
Please join us at our administrative office in Doylestown, PA for an Education Program from Rachele Daniels, our Medical Educator, on the services provided at AWP, the dynamics of Domestic Violence, and health impacts of Domestic Violence. Next, we will be sorting and organizing our pantry, which supplies essential items to our clients, such as toiletries, cleaning supplies and diapers. Tasks will include labeling and weighing, reshelving and discarding. (Climbing stairs and carrying up to 30 pounds required).

AWP provides a full range of programs and services to individuals experiencing domestic violence, their children, and the community. Woods System of Care: Integrated Care Model
Woods System of Care offers participants a tour around the Woods Services property in Langhorne, PA, which is home to 475 residents. Woods Services is an affiliate of the Woods System of Care. This tour will provide insight into some of the innovative programs that the Woods System of Care has implemented to improve client care and health outcomes. Conference participants will visit the Medical Center at Woods, Penn Dental Medicine at Woods, the Center for Behavioral Health, and the recreation team, which includes a barn and nature trail.

* The Medical Center is an on-site center that provides comprehensive medical care for patients with disabilities.
* Woods has partnered with the University of Pennsylvania School of Dental Medicine—one of the leading dental schools in the United States—to provide access to dentists for kids with autism and other special needs, and for adults with disabilities.
* The Center for Behavioral Health specializes in delivering comprehensive behavioral healthcare that draws on a wide range of therapies, interventions, and support systems to provide thorough assessment, diagnosis, treatment, prevention, and follow-up care.
* The recreation team visit includes a visit to our barn, which is home to our many farm animals, as well as a walk down the nature trail, where residents enjoy to connect with nature and their environment.

Beechwood NeuroRehab: Comprehensive Care After Brain Injury
This will be an immersive, hands-on experience as attendees spend a few hours walking in the shoes of individuals with brain injuries and explore the challenges and the triumphs of these individuals as they continue to adapt to their lives with a new and different set of abilities.

Beechwood NeuroRehab is a program that provides individuals with skilled rehabilitation across many stages of their lives. Structured as a post-acute residential and outpatient program, Beechwood NeuroRehab offers occupational, physical, speech-language, neuropsychology, and cognitive rehabilitation therapies as well as structured day programming and residential services for individuals with brain injuries and other neurological conditions.
Most importantly, Beechwood NeuroRehab utilizes collaborative treatment plans within each participant’s individualized interdisciplinary team to provide them with a variety of interventions to improve their quality of life as this in turn fuels their motivation to remain connected with their lives.
During your visit, you will have the chance to immerse yourself in different aspects of these individuals’ lived experiences such as attending various therapies or joining individuals in their structured day program work units. One hour you may be learning to use equipment to treat physical disabilities such as hemiplegia or visual impairments, and the next you may be assisting individuals in the horticulture or culinary units to ensure they fulfill their work-ordered day. Through our holistic treatment approach focusing on treating the whole person at each stage of their life as they recover and redefine their lives post-injury, we are supporting some of the following areas:
· Hemiplegia with and without spasticity
· Visual impairments
· Balance
· Executive and cognitive functioning
· Work related training
· Mental health

As part of the Conference on the Move at TUFH 2025, participants will get an up-close look at how cutting-edge healthcare models are revolutionizing access to care. Hosted by CCA, this engaging session will feature a lively panel discussion on the transformative impact of convenient care clinics, including retail health, mobile health, and virtual care models, as they mobilize across the U.S. to enhance accessibility, affordability, and quality of care. TUFH 2025 delegates will also have the opportunity to visit a retail clinic.
CCA is the international trade association for convenient care clinics, represents clinics commonly found in pharmacies, supermarkets, and other easy-to-access locations. These clinics prioritize patient-centered care, focusing on prevention, chronic disease management, and tackling healthcare disparities head-on.

Butler Ave., Doylestown, PA 18901—a collaborative initiative between Bucks County Opportunity Council (BCOC) and Delaware Valley University, dedicated to providing fresh, local produce to the anti-hunger network.
